De acordo com especialistas, a maior parte da popula\u00e7\u00e3o mundial n\u00e3o a recebe em quantidade suficiente para manter uma massa \u00f3ssea saud\u00e1vel e reduzir ao m\u00ednimo o risco de fraturas. Para determinar se o paciente conta com a quantidade adequada de vitamina D no organismo, mede-se a concentra\u00e7\u00e3o s\u00e9rica do metabolito 25-hidroxivitamina D, que \u00e9 o que melhor determina os dep\u00f3sitos de vitamina D no organismo.<\/p>\n<p style=\"text-align: justify\"><!--more-->Sobre os n\u00edveis de vitamina D recomendados, da Funda\u00e7\u00e3o Internacional pela Osteoporose recomenda que as pessoas idosas mantenham n\u00edveis de 25-hidroxivitamina D de 30 ng por ml, indicando que em alguns casos seria necess\u00e1rio que a pessoa consuma at\u00e9 2000 UI por dia para alcan\u00e7ar o n\u00edvel recomendado. Por outro lado, o Institute of Medicine (IOM) assegura uma concentra\u00e7\u00e3o s\u00e9rica de 20 ng por ml 25-hidroxivitamina D protegeria 97,5% da popula\u00e7\u00e3o contra problemas \u00f3sseos como fraturas e quedas.<\/p>\n<p style=\"text-align: justify\">Em pacientes idosos recomenda-se um programa de exerc\u00edcios e um consumo de 1200mg de c\u00e1lcio por dia. O IOM recomenda 6000IU di\u00e1rios para mulheres na p\u00f3s-menopausa e 800IU di\u00e1rios para pessoas com alto risco de osteoporose ou de idade superior a 70 anos. Todavia, n\u00e3o foi estabelecido a dose \u00f3tima de vitamina D, sabe-se que, em geral, por cada 100IU de vitamina D, a concentra\u00e7\u00e3o s\u00e9rica de 25-hidroxivitamina aumenta 1 ng por ml (3nmol por 1), na maioria dos casos, para reduzir o risco de fraturas s\u00e3o empregadas doses di\u00e1rias de vitamina D de 400 a 100IU. Nos Estados Unidos est\u00e1 sendo realizado um estudo que busca analisar os efeitos da vitamina D para a preven\u00e7\u00e3o do c\u00e2ncer e de doen\u00e7as cardiovasculares.<\/p>\n<p style=\"text-align: justify\">Diversos estudos demonstram que existe uma associa\u00e7\u00e3o entre baixos n\u00edveis de 25-hidroxivitamina D e um aumento na frequ\u00eancia de fraturas. Em um estudo realizado com homens idosos, os n\u00edveis do metabolito abaixo dos 16mg por ml (40nmol por 1) se associara a um maior risco de fraturas.
